Prohibited Employment Declaration Form At the time of registration, all volunteers, both minors and adults, will be asked to sign a copy of the Prohibited Employment Declaration. This will be handed out at the time of registration, or may be downloaded here. We have been informed that under legislation sporting clubs are required to ensure any volunteers sign the declaration and it must be held on file by the club for a period of 2 years after that person ceases all involvement with the club. Minors who volunteer will be asked to co-sign with a parent/guardian. If you do assist either regularly or even on occassions at swim carnivals, please sign the form. |
